Service Description

Two weeks to stabilize and support you through a major transition. Something Like: +New cancer diagnosis + chemo scheduling +Hospice transition +Sudden discharge from hospital +Navigating rehab placement +Prepping for surgery or major treatment Includes: +Up to 10 hours over 2 weeks +Liaising with providers, payers, and family +Attendance on key calls +Coordinating next steps Typically we can support in 5-8 hours. Additional hours at $175/hour if needed.
